We, ____________________________ (hereinafter referred to as the “___________”) and _______________________ (hereinafter referred to as the “_______________”) hereby voluntarily and conscientiously DECLARE AND AGREE THAT: –

1. The parties herein acknowledge that they are the biological parents of

The parties have freely and voluntarily entered into this agreement and fully understand and appreciate its contents, effect, true purpose and intent relating to their respective rights, duties, responsibility and obligations under this agreement and in Law as regards to the child and as between each other.
3. The parties acknowledge that they have equal shared parental responsibility over the child in the manner and to the extent envisaged by all the applicable laws.
4. _______________ shall stay with the Child at the location ________________________ for a period of six (6) months from the date of signing hereof and shall be amended/extended accordingly as parties may agree.
5. ________________ may have visitation and access rights to the child on modalities to be agreed on between the parties from time to time and as circumstances may dictate.
6. ___________________ shall inform ___________________ on their physical location in any event that they are vacating from the provided address stated at No. 4 above.
7. ____________________ shall bear all the cost of the needs of the child.
8. The terms of this agreement and/or with any variations thereof shall remain valid up to and including the child’s 18^th

Any provision of this Agreement which is found to be prohibited or unenforceable by (an arbitrator or court) for any reason whatsoever shall, as to such prohibition or unenforceability, be ineffective and deemed as deleted to the extent of such prohibition or unenforceability, without invalidating or prejudicing the remaining provisions hereof. Any such prohibition or unenforceability shall therefore not invalidate or render unenforceable the rest of the provisions of this Agreement.
10. The rights which each of the parties has under this agreement shall not be prejudiced or restricted by any indulgence or forbearance extended to another party. No waiver by any party in respect of a breach shall operate as a waiver in respect of any subsequent breach.
11. This agreement embodies the entire agreement between the parties hereto. No amendment or variation of any of the provisions of this Agreement shall be of any force or effect unless reduced into to writing with prior consent of by both parties.
12. The parties shall make every effort to settle amicably by direct informal negotiation any disagreement or dispute arising between them or in connection with this Agreement. In the event that the parties fail to reach an agreement, all disputes, differences and questions of any nature which at any time arise between the parties or any of them out of the construction of or concerning anything contained in or arising out of or in connection with this Agreement or as to the rights, duties or liabilities under it of the parties to it shall be shall be referred to a court of competent jurisdiction for the time being dealing with matters relating to parental responsibilities and rights of the child.
