RELEASE FORM.
I, __________________, Contact Info: ______________, Contracted by __________ Contac Info: _____________________, for a dance project, hereby grant and authorize SOUNDFEST Contact Info: _____________the right to take, edit, alter, copy, publish or make use of my photos and/or video of my dance moves/choreography in a lawful purpose. I acknowledge that I have consented and willingly provided SOUNDFEST permission to use and reuse my likeness in the music video titled ______________.
The authorization shall extend indefinitely, unless I request for SOUNDFEST to cease from further use of my likeness before the production and official launch of the music video.
I understand that my likeness as shall be used in the production of the music video of SOUNDFEST shall become their property. SOUNDFEST shall be the sole owner of the music video and my likeness as used in the music video and shall have and enjoy all and any proprietary right over the music video indefinitely.
I acknowledge that I have agreed for my likeness to be used without any compensation. I waive any and all right to royalties arising and/or related to the use of my likeness. SOUNDFEST shall give referral of my work/chorography to any prospective employer.
I hereby hold harmless and release SOUNDFEST from any and all liability, real or imagined, legal or otherwise, petitions and causes of action in which, I or my representatives, executors, administrators, and/or any other person may make while acting on my behalf.
I agree that in the event of disputes, SOUNDFEST and I shall use arbitration to solve the matters. This release form is governed by and construed in accordance with the laws of the State of New York, USA. Exclusive jurisdiction and venue shall be in New York, USA.
