I, Eric Freeze, Claimant, do hereby claim a lien under the Uniform Commercial Code – 1 and hereby aver as follows:

  1. The property mandatory to be charged with the lien is located at 47796 and 47972 Moen Road, Concrete, Washington 98237 (the “Property”).
  2. The name of the owner of the above property is the Ronald L. Freeze Revocable Trust and the Ann G. Freeze Revocable Trust currently represented by Elizabeth E. Gallagher.
  3. Claimant is payable a sum of $740,000.00.
  4. Claimant rendered services to Owner. Owner promised and agreed to provide indefinite residency for services rendered but has failed to do so despite Claimant’s multiple requests.
  5. The affidavit requires Owner to pay to Claimant a sum of $740,000.00. Claimant has not received such consideration from Owner.
  6. The above property shall be charged with the lien until payment of consideration in the sum of $740,000.00 by Owner to Claimant.
