I, EF, Claimant, do hereby claim a lien under the Uniform Commercial Code – 1 and hereby aver as follows:

    1. The property mandatory to be charged with the lien is located at [Location address withheld] (the “Property”).

    1. The name of the owner of the above property is the RF Revocable Trust and the AF Revocable Trust currently represented by EG.

    1. Claimant is payable a sum of $740,000.00.

    1. Claimant rendered services to Owner. Owner promised and agreed to provide indefinite residency for services rendered but has failed to do so despite Claimant’s multiple requests.

    1. The affidavit requires Owner to pay to Claimant a sum of $740,000.00. Claimant has not received such consideration from Owner.

    1. The above property shall be charged with the lien until payment of consideration in the sum of $740,000.00 by Owner to Claimant.
