SERVICE DENIAL FORM
I, the customer herein, hereby agree that Nexus Technology Provider, LLC has offered a support contract agreement with their IT Managed Services.
However, I do not wish to receive the provided services and hereby refuse the backup services for our software and devices.
I hereby waive any claims which may arise now and, in the future, against Nexus Technology Provider, LLC and release them from all liability, and agree to indemnify, defend and hold them, its officers, agents, representatives, or employees against any and all injury, product damage, or loss sustained as a result of my refusal to the services.
In signing this service denial, I am not relying on any oral, written, or visual representations or statements by the Nexus Technology Provider LLC, or any other inducement or coercion.
I confirm that, by my own free will, I have read and understood the Terms and Conditions of this service denial form before signing it and agree that this service and denial form is fair. The terms and conditions of this Agreement shall apply to successors, assigns, and affiliates of all parties.
