SERVICE AGREEMENT

Thank you for choosing Second Chance services to assist you with obtaining a “Clean Slate”. We know the barriers that are associated with having a criminal record and the need to give those seeking economic growth the opportunity to do so without restrictions.

HOW IT WORKS.

You will meet with your Expungement consultant twice. The first time is a preparation meeting and the second is the actual Court date. An appearance or hearing is not necessary for the process but periodically, the judge or his administration will request that you come in to make your case in person. This is very rare but possible!

To obtain a pardon, your appearance is mandatory. You will have to go before a panel of 12 people to make your case as to why you are deserving of your rights back.

NOTE**In order to be eligible to have a record expunged or a Pardon in Wisconsin,

ALL of the conditions of your sentence must be met and you must have completed probation successfully with no revocations and no open/pending cases.

All your fines, fees, and restitution must be paid to the state BEFORE proceeding.

Not doing so can result in your expungement/ Pardon being denied indefinitely.

For a pardon- you have you wait 5 years before applying with no open or pending cases.

Before you get started on your road to a “Fresh Start”, here’s a list of items you’ll need to begin the Record expungement or the “rights restoration” process better known as obtaining a pardon by the Governor.

To begin the process you will need:

  1. A letter to your sentencing judge requesting your court case expunged.
  2. 2 or more character witness letters for supporters, (can be from parents, clergy, coworkers or supervisors from work or a spouse)
  3. Any awards, certificates, or achievements that you’ve made since being incarcerated.
  4. And any other conditions that were a part of your sentence such as community service, attending anger management and/or AODA, obtaining a high school diploma, etc.  These stipulations must be met prior to filing.
  5. Felony cases will need their certificate of discharge from the Department of Community Corrections.
  6. Any other necessary documents with be provided to you.

Please check the boxes and sign if you agree to the terms and process.

I have read and understood this document to the best of my abilities and the information that I have provided is true according to my understanding.

I understand that it’s a possible that despite the best efforts of Clean Slate Milwaukee and the second Chance program, myself, and my supporters, that ultimately the decision of my sentencing falls on the judge and that he/she has the final say on expunging my record.
