Dear Board Members,
On or about July 18, 2022, the Georgia Board of Nursing sent me a request for a sworn
statement regarding a complaint that was filed against me. The complaint alleged inter alia, that
I had submitted a TB Quantiferon lab result that the date was altered and a COVID vaccination
card with the name digitally altered. I hereby deny the complaint and all allegations therein, in its
entirety. Notably, it would pointless for me to submit false information to NLB Healthcare, when
I have an active RN and unrestricted license, and unexpired credentials. Upon information and
belief, I aver that my ex-husband, Charles Cook (hereinafter “Cook”) is responsible for the
alterations. I have had a tumultuous relationship with my Cook. Besides, he severally threatened
to end my career. This is not the first time he is altering my information. Below, I have provided
a previous instance when Cook altered my details when I was applying for a job.
I. Background
Cook and I divorced in 2018. However, in 2021, Cook became gravely ill, thus
necessitating me to reconcile with him, upon which Cook would spend time on my house. That
notwithstanding, tension remained in our relationship, and the marriage had not been rekindled.
On or about May 15, 2022, after expressing to Cook my intention to secure a second job, he
encouraged me to apply for a job at Jackson Nurse Professional. Accordingly, I applied for the
job using my legal name “Shana Marie McDade”, and submitted my driver’s license and ACLS
card in support thereof. However, on June 2, 2022, I received a communication from the Jackson
Nurse recruiter asking if I ever used “Shana Lavonne Cook”. In response, I informed the
recruiter that Marie Cook was my name before divorce and after the divorce; I changed my name
back to “Shana Marie McDade”. I then sent the recruiter a copy of the divorce decree to prove
my averments. Interestingly, the email account that I used to communicate to the Jackson
recruiter was suspended due to suspicious activity, which can be attributed to Cook.
On July 18, 2022, I had to file a police report against Cook, for using my information
fraudulently (Exhibit- Police Report).
It is noteworthy that Cook had access to my laptop and email. He also set up my private
duty nurse website, domain, and email addresses. Cook has technology skills with an impeccable

academic background. He is a summa cum laude graduate of the California Institute of
Technology with a Ph.D. in Electrical and Electronics Engineering. Besides, he has worked in
various capacities as a professor, electrician, and president of a company. Cook therefore has the
necessary technological skills to effect the alteration of the information that was submitted to
NLB Healthcare.
In light of the foregoing, the allegations charged against me are another instance of
Cook’s protracted actions, in a bid to see my ruin. I therefore maintain that I am completely not
responsible for the alleged conduct. I am a victim of the ill-willed conduct of my ex-husband. I
further assert that I am in perfect shape to continue my nursing practice without any license
restrictions.
Further, throughout my nursing practice that started in 2013, I have faithfully and
dutifully carried all of my obligations and/or duties with utmost care and professionalism. I also
have impeccable leadership skills, and have served in leadership capacities in my community.
For instance, from 2013 to 2015, I served as the President of a non-profit in Cleveland, Ohio.
II. The incident

On or about June 17, 2022, NLB Agency went to the testing center to get a Lab test done. I was
told that the TB Quantiferon test could not be more than 12 weeks old. Accordingly, I
