MEMBERSHIP AGREEMENT.
This Membership Agreement (hereinafter referred to as the “Agreement”) is made and entered on this ____ day of _____ 2021 (the “Effective Date”), between The Bunker Fitness, Contact Info: 350 Silas Deane Highway, Wethersfield CT, 06109 Website www.thebunkerfitness.com (860) 704-9465 (hereinafter referred to as the “Company”), and ___________________________________, Contact Info: _____________________ (herein referred to as the “Member”).
NOW THEREOF, in consideration of the mutual covenants and promises made by the Parties hereto, the Parties covenant and agree as follows:
- TERM.
The initial term of this Agreement shall be for the period of _____________ Months commencing on the Effective Date herein.
- SCOPE.
The Member shall have unlimited access to compression and red-light sessions, and benefits of 10% off any add-ons such as percussion sessions and/or rock tapering.

The Member understands that sessions are limited to booking one session per day, and they must schedule the session in advance with a service provider.
- FEES/PAYMENT.
Member shall pay a start-up fees of Twenty Dollars ($20) payable on the Effective Date herein, and a monthly fee of Fifty Nine Dollars and Ninety Nine Cents ($59.99).
- CANCELLATION.
This Membership shall automatically be cancelled upon the expiry of the term herein, however the Member can keep making monthly payments to keep having access to services.
- LIABILITY/INDEMNITY.
Each Member understands the risks involved with the services covered under this Agreement and they acknowledge that it is their sole responsibility to exercise caution while engaging these services. The Company shall not, during the term of this Agreement, be held liable for any injuries sustained by any Member while the Member is engaging and enjoying the services covered herein. The Member agrees to indemnify the Company against any injury claims that may arise from this Agreement.
- MODIFICATION.
This Agreement contains the entire Agreement between the Parties relating to the subject matter hereof and supersedes any and all prior agreements or understandings, written or oral, between the parties related to the subject matter hereof. No modification of this Agreement shall be valid unless made in writing and signed by both parties hereto.
- GOVERNING LAW AND JURISDICTION.
This Agreement shall be governed by and construed in accordance with the laws of the State of ______________, USA. Exclusive jurisdiction and venue shall be in ____________, USA
Each party represents and warrants to the other that such party has acted in good faith, and agrees to continue to so act, in the negotiation, execution, delivery, performance, and any termination of this Agreement.
