LIABILITY WAIVER
I ______________________ (Name of the Parent) do hereby release, forever discharge, and agree to hold The Hero Within You Network harmless from any and all liability, claims, demands, lawsuits, and expenses arising from personal injury, sickness, death, or property damage of any nature whatsoever, which may be incurred by the participant (under 18 or 18 and older) while attending the program activities.
Further, I hereby assume all risk of personal injury, sickness, death, damage and expenses arising from the participant’s participation in all activities, including recreation and work activities.
I hereby agree to furnish the participant with the necessary transportation, food, and medical treatment.
I release The Hero Within You Network of all responsibility and consequences that may arise as a result of any injury suffered and resulting treatment. Further, I agree to accept any and all financial responsibility as a result of schedule, acquiring and/or providing medical treatment.
I hereby agree to indemnify and hold The Hero Within You Network harmless from and against any and all claims, demands, actions, lawsuits, damages and liabilities, including attorney’s fees and expenses sustained as a result of the participant’s negligent, willful, or intentional acts.
I hereby acknowledge that I have read this entire document, that I understand its terms and provisions, that I understand it affects my legal rights as well as, if applicable, those of my child, and that it is a binding agreement.
