INSURANCE AGREEMENT

Definitions:
Marketplace – U.S. Government Healthcare system is a resource where individuals,
families, and small businesses can: compare health insurance plans for coverage and
affordability, get answers to questions about your health care insurance, and find out if
you are eligible for tax credits for private insurance or health programs like Medicaid or
the Children’s Health Insurance Program (CHIP), enroll in a health insurance plan that
meets your needs. The Marketplace can be accessed at: www.healthcare.gov.
The Insurance Guide, LLC – TIG – a XXX LLC company with Doc. No.:
XXX.
Customer – The customer – is a purchaser of the health/life/travel/property/auto
insurance through connection with ‘The Insurance Guide’ through phone, website, e-
mail, and other means.
U.S. Government Healthcare system – www.healthcare.gov
Healthcare.gov – U.S. Government Healthcare system
CMS – Healthcare.gov
Life/travel products require an Agent to be licensed in life insurance. The product does
not include health products, which require a health license.
Quotit Corporation – life insurance provider by National General company.
Notes:
The Insurance Guide, LLC is not a U.S. government healthcare system. It’s a name
representing a licensed and Federally Facilitated Marketplace (FFM) certified insurance
agency (in the State of Florida) and agent licensed in the State of Business to transact
insurance to customers. CMS licenses the agent.
1. Binding Contract:

The terms of this agreement are a binding, enforceable contract between The Insurance
Guide, LLC and the customer buying health/life insurance.
The Insurance Guide, LLC agrees to provide information on health and life/travel
products to the customer(s).
2. Financial Arrangements:
The client agrees to a billing period established at the time of purchase (Monthly,
Quarterly, Semi-Annual or Annual) beginning upon the Marketplace’s health carrier and
Quotit Corporation’s delivery date of order.
Initially selected billing period payment and setup charges, if any, shall be due upon
receipt of the contract.
This agreement will automatically renew for successive months (based on the initial
billing period selected at the time of purchase) unless canceled in writing thirty days
prior to the renewal date.
The client will receive an invoice for charges, and the payment is due upon receipt.
Renewal prices are subject to change. Renewal of services by Client indicates
agreement to Contract revisions.
3. Failure to Pay for Services:
Any account with outstanding fees of 45 days or more will be suspended for non-
payment without notice.
Accounts will be reactivated upon payment of any outstanding charges plus applicable
re-activation fees. Re-activation fee is up to $199.00.
Deactivated accounts will incur a one-time charge of up to $199.00 to the customer 15
days after the suspension.

4. Cancellations:

To cancel a life/travel product, the client agrees to email The Insurance Guide, LLC:
info@gotig.net with the subject ‘Cancel,’ In the text box, add your name, phone number,
state, and the protection you acquired, and the reason for cancelation.
Cancellations of any life/travel/property/auto products will have to be made no more
than 15 days from the day of purchase. These cancellations will not incur any fees.
Cancellations made after the 15 days from the time of purchase may incur a one-time,
up to $199.00 non-refundable fee from The Insurance Guide, LLC.
The customer agrees that all parts of this agreement were made available to read and
print. The signature confirms all aspects of the contract to be enforceable at The
Insurance Guide, LLC’s discretion.
5. Material and Products:
The client will provide The Insurance Guide, LLC with material and data in an accurate
and truthful condition. This data will be used to complete health and
life/travel/property/auto insurance applications. Any discrepancies will be the sole
responsibility of the customer.
The client agrees that it is not the responsibility of The Insurance Guide, LLC to provide
Customer Support outside of the defined service of The Insurance Guide, LLC.
6. Warranties:
The Insurance Guide, LLC will exercise no control over the content of the information
passing through the network.
The Insurance Guide, LLC makes no warranties or representations of any kind, whether
expressed or implied, for its service.
The Insurance Guide, LLC also disclaims any warranty of merchantability or fitness for a
particular purpose and will not be responsible for any damages that may be suffered by
the Client, including loss of data resulting from delays, non-deliveries, or service
interruptions by any cause or errors or omissions of the Client.

Use of any information obtained by The Insurance Guide, LLC is at the Client’s own risk.
The Insurance Guide, LLC specifically denies any responsibility for the accuracy or
quality of information obtained through its services.
Connection speed represents the speed of a connection to and does not represent
guarantees of available end-to-end bandwidth.
The Insurance Guide, LLC expressly limits its damages to the Client for any non-
accessibility time or other downtimes to the prorate monthly charge during the system
unavailability.
The Insurance Guide, LLC specifically denies any responsibilities for any damages
arising due to such unavailability.
Suppose this material is not "Server-ready," The Insurance Guide. In that case, LLC
may, at its option and at any time, reject this material, including but not limited to after it
has been put on The Insurance Guide, LLC.
7. Privacy:
The Insurance Guide, LLC considers the Clients’ Data and the content of all Client
databases to be private.
8. Age:
The Customer certifies that they are at least 18 years of age.
9. Termination:
Neither party may terminate this Agreement without cause by giving the other party 30
days’ written notice. Notwithstanding the above, The Insurance Guide, LLC may
terminate service under this Agreement at any time, without penalty, if the Client fails to
comply with the terms of this Agreement.
10. Limited Liability:
The client’s exclusive remedies for all damages, losses, and causes of action, whether
in contract, tort, negligence, or otherwise, shall not exceed the Client’s aggregate dollar
amount during this Agreement and any reasonable attorney’s fee and court costs.

11. Indemnification:
The client agrees that it shall defend, indemnify, save and hold The Insurance Guide,
LLC harmless from any demands, liabilities, losses, costs, and claims, including
reasonable attorneys’ fees ("Liabilities") asserted against The Insurance Guide, LLC, its
agents, its customers, servant’s officers, and employees, that may arise or result from
any service provided or performed or agreed to be performed or any product sold by
Client, its agents, employees or assigns.
The client agrees to defend, indemnify and hold harmless The Insurance Guide, LLC
against Liabilities arising out of;
i. Any injury to a person or property caused by any products sold or otherwise
distributed in connection to The Insurance Guide, LLC services;
ii. Any material supplied by the Client infringing or allegedly infringing on the
proprietary rights of a third party;
iii. Copyright infringement and
iv. Any defective product that the client sold on Quotit Corporation Server.
12. Contract Revisions:
Revisions to this Contract will apply to previous Contracts. Modifications will be
considered agreed to by the Client on renewal of The Insurance Guide, LLC once the
same is in writing and signed by both.
13. Data Usage:
The customer consents to release the conversation recorded in purchasing health,
life/travel insurance to The Insurance Guide, LLC to use anywhere, anytime.
The customer agrees to hold The Insurance Guide, LLC harmless if the recorded
conversation is shared with an investigative body and allows the recording of the
conversations solely focused on purchasing insurance with The Insurance Guide, LLC.
14. Arbitration:

The customer agrees to notify The Insurance Guide, LLC 30-days after the delivery of
any action against the company. These include methods of email to: info@gotig.net and
sending a letter to:

The notification should include any issues/problems.
The customer should have the utmost effort in detailing to the best of their ability.
Suppose any civil or public action is taken before notification, The Insurance Guide. In
that case, LLC reserves the right to sue for breaking the arbitration agreement and
collect any damages and attorney fees from the customer.
Any contract binding disputes will be referred to Hillsborough County in the state of
Florida.
15. Entire Agreement:
This Agreement constitutes the parties’ entire understanding and supersedes any prior
written or oral agreements.

16. Governing Law:
This Agreement shall be governed and construed in accordance with the laws of the
State of XXX.
THE CUSTOMER AGREES THAT ALL PARTS OF THIS AGREEMENT WERE MADE
AVAILABLE TO READ, PRINT AND THAT THE SIGNATURE IS A CONFIRMATION
FOR ALL PARTS OF THE CONTRACT TO BE ENFORCEABLE AT THE
INSURANCE GUIDE, LLC’S DISCRETION.
