EMERGENCY MEDICAL AUTHORIZATION.
I, Kathryn A Wilson (hereafter “Parent and Legal Guardian”) as mother to 17-year-old Evan R. Wong (hereafter the “Minor”), hereby grant to Ivan Batsanov and Veleslava Zhereva (together as “Joint Temporary Medical Guardians”) in their capacity as coaches of the Lolani Swim Club the COMPLETE AUTHORITY, such as I myself would have been entitled to exercise in my own capacity, over any decisions covering Evan’s emergency and other medical care that may arise in the period between 2nd JUNE 2024 and 11th JUNE 2024 (the “Duration of the Guardianship”) when the Minor and other members of the Lolani Swim Club will be touring Australia.
The Joint Temporary Medical Guardians agree to at all times in the duration of the guardianship exercise the medical guardianship granted by the Parent and Legal Guardian reasonably, responsibly and in the best interests of the Minor.
It is hereby agreed.
