Atlas Behavior Agency LLC Job Description     JOB TITLE:  Health Service Provider in Psychology   GENERAL JOB DESCRIPTION:    To attest that the correct diagnosis is in place, with the appropriate functional analysis, with valid behavioral data and that behavioral data is collected.  To determine if appropriate psychotropic usage is at levels which are appropriate to diagnosis.  To sign the behavior plan at least annually.   SUPERVISORS:  Executive Director   NATURE AND SCOPE OF JOB:   Relationships   Atlas Behavior Agency LLC is a programming facility and as the Health Service Provider in Psychology, it is important to realize the role as an integral part of the total system.  Since the ultimate goal is to improve quality of life, each organization must work together to provide the ultimate in individualized programming. All staff must work together as one team to ensure individual rights and to meet individual needs.   Qualifications, Skills, Knowledge, and Abilities   The Health Service Provider in Psychology will have the following qualifications:             Interest in the commitment to serve and improve the quality of life for individuals with developmental, physical or mental disabilities. Kind, compassionate, caring, and empathetic. Of good moral character. Have a doctoral degree in clinical psychology; counseling psychology, school psychology, or another applied health service area of psychology. Is licensed in the State of Indiana. Has at least two (2) years of experience in a supervised health service setting in which one (1) year of experience was obtained in an organized health service training program and in which at least one (1) year of experience was obtained after the individual received the individual’s doctoral degree in psychology. Comply with continuing education requirements under IC25-32-2. Be certified as a Health Service Provided in Psychology with the Division of Disability, Aging and Rehabilitative Services (for waiver services) and by the Bureau of Developmental Disabilities Services (for BDDS specific services). Must be 21 years of age or older. Current C.P.R. and First Aid Certification. Negative TB (Mantoux) Test or chest x-ray. Criminal background check shows no history of: a.       Abuse or fraud in any setting. b.      Substantial or repeated violations in the care of disabled persons c.       Conviction of a crime related to the disabled populations d.      Substantial and repeated violations in the operation of a residential or health care facility. Current Driver’s License and proper vehicle insurance.
