RE: ATTESTATION OF COURT CASE(S) FOR MEDICAID SERVICES
I am Tinesha Boles of ID. Number [ENTER IDENTIFICATION]. The purpose of this letter is to make an attestation of court cases that I have been involved in, for the purposes of my placement into your Medicaid program.
Accordingly, I shall proceed to explain what happened in every case.
Close to twenty years ago, I had misplaced my check book and went into the bank to report the checks lost. The bank issued me a new checkbook and did not realize that the account number was not changed. Notably, the checks had the same account information on them. So, I wrote checks but did not pay attention to them not hitting my account. It follows; the checks bounced. I must admit that I was young, and I did not know how to manage a bank account. By the time I realized the checks had bounced, I no longer had the money in the account to pay the stores back. So, it took some time for me to re-pay the money back.
The following are the Court cases brought against me for the said bounced checks:
- Case Number 02PCN000267-0000
On or about February 6, 2002, I was charged with theft (misdemeanor) following a bounced cheque that I issued. The Court entered a final disposition on or about February 21, 2002 with a sentence of Criminal Conversion.
- Case Number 02CRN000316-0000
On or about February 21, 2002, I was again charged with theft (misdemeanor) also following a bounced cheque that I issued. The Court entered a final disposition on or about August 26, 2003, where my case was dismissed.
- Case Number 01PCN001086-0000
On or about October 22, 2001, I was charged with theft (misdemeanor) following a bounced cheque that I issued. The Court entered a disposition on or about November 5, 2001 with a sentence of Criminal Conversion.
