Shapiro Zwanetz & Associate (“the Firm”),

and

CONCERNING

 

CONCIERGE LEGAL SERVICES (“the Services”).

 

  • Client agrees to place a credit card to be billed at $600 per hour (“Fee”) for:
  • Phone calls
  • Text messages
  • Video Conferencing
  • Social media consultations  
  • This Section applies to such services done outside the 8:45 a.m. -5:45 p.m. Monday to Friday period (“business hours”). 


  • For each 24-hour period of any participant in the Services, the initial contact will be billed at a minimum of fifteen (15) minutes and each additional minute will be billed with an accurate calculation put in.  
  • All other contacts between the Client and the Firm during the usual business hours will not be billed as part of the Services herein. Such other contacts are covered under the flat fee charged on all criminal matters in the Retainer Agreement.  
  • The Client wo uses the Services must have signed the Retainer Agreement.

  1. I hereby authorize Shapiro Zwanetz & Associates to automatically debit my saving/checking/credit card account with the below-mentioned payments on the below-mentioned dates. This authority is to remain in full force and effect until Shapiro Zwanetz & Associates has received written notification from me of its termination in such time and manner as to afford Shapiro Zwanetz & Associates a reasonable opportunity to act on it.

  1. The signature on this Rider permits the Firm to charge the credit card listed in the Card Authorization above (Provision 5(a)) in the amounts and on the dates listed in the event ANY fee payment is NOT made by Client by cash or check prior to the applicable Fee Payment Date. Client acknowledges and agrees that if Client has chosen to pay the Fee pursuant to a Payment Plan, it is the Client’s responsibility to inform the Firm prior to the next payment due date if the credit card listed on the Credit Card Authorization is no longer active or has an insufficient limit for the next fee payment amount to be charged by the Firm. In that event, Client agrees to execute a substitute Credit Card Authorization Form prior to the next Fee payment date or action will commence pursuant to the provisions in Sections 2 (d) and (e) below.  
  2. Client agrees to pay ALL unpaid portions of the Fee IN FULL IN CASH OR BANK CHECK WITHIN 24 HOURS of being notified by the Firm that ANY credit card charge made by the Firm in accordance with the Credit Card Authorization Form then in effect is reversed by the applicable credit card company. 
  3. Client acknowledges and agrees that the Firm may withdraw from representation and terminate this agreement in the event Client breaches the provisions of any provision in this Section 5.  
  4. For purposes of this Rider, “Trial Date” is defined as the first court appearance made on Client’s behalf by the Firm. CLIENT UNDERSTANDS AND AGREES THAT THE FIRM SHALL NOT REPRESENT THE CLIENT OR APPEAR ON CLIENT’S BEHALF IN COURT IF THE ENTIRE FEE HAS NOT BEEN PAID IN FULL AT LEAST FOURTEEN (14) DAYS PRIOR TO THE TRIAL DATE.
