This letter serves as my formal request for details regarding my payment of sanctions. On or
about January 9, 2023, I filed a Motion for Sanctions against Defendant(s) in the referenced
case. Consequently, the Court issued an Order sanctioning me for filing a meritless motion.
I intend to pay the sanctioned amount through a check. However, I do not know who to make
out the check to. I also do not have the address for the recipient of the check.
You should note that the check shall be from my disability funds. The disability was triggered
during my time at Southern California Permanente Medical Group (SCPMG), where I was
subjected to discrimination, oppression, malice, and fraud. It is therefore unfortunate that said
Defendant seeks my disability money.
Accordingly, I request you provide:
1. The identity of the recipient of the monetary sanctions;
2. Address for service for said individual/entity.
