ADDITIONAL SERVICE AGREEMENT

The undersigned ______________________________ (the “Customer”), as a patron of the UPS Store located at ______________________________ hereby agrees that:

Services Provided

  1. The Customer hereby agrees to engage UPS to provide the Customer with services (the “Services”) under the following rates:
  • UPS will provide the Customer with mailbox services for up to 15 packages at an extra cost of $4.00 for every package received at the store.
  • A fee of $10.00 will be charged for every package over 35lbs.
  • A fee of $20.00 will be charged for every package over 60lbs.
  • A fee of $10.00 will be charged to re-enter and/or close a package to be placed back in the warehouse. 
  1. UPS hereby agrees to provide such Services to the Customer.

Payment

  1. Payment for services provided will be done at the end of each month. UPS can keep a credit card authorization or the Customer can pay in person.

Term of Agreement

  1. The term of this Agreement (the “Term”) will begin on the date of this Agreement and will remain in full force and effect indefinitely until the original contract for general mailbox services is terminated.
  2. Except as otherwise provided in this Agreement, the obligations of UPS will end upon the termination of this Agreement.

IN WITNESS WHEREOF the Parties have duly affixed their signatures under hand and seal on this 2nd day of October, 2020.
